超声内镜在胰周液体积聚诊断与治疗中的研究进展

摘要

胰周液体积聚(PFCs)是急性胰腺炎常见并发症,其精准管理对预后至关重要。超声内镜(EUS)凭借高分辨率成像与实时介入能力,已成为PFCs诊疗的核心工具。本文系统综述EUS在PFCs分类诊断(依据2012年修订版亚特兰大标准)、技术优势(如微创引流、血管规避能力)及治疗策略中的进展。研究显示,EUS在区分假性囊肿(PPC)与包裹性坏死(WON)方面准确性显著优于CT(89.6% vs. 13.7%),且EUS引导引流技术成功率高达93.9%~97.4%,住院时间短、并发症率低。新型支架(如LAMS)的运用进一步优化了疗效。尽管面临设备成本高、操作依赖经验等挑战,EUS通过多技术融合(如EUS-MRI)及规范化培训持续提升临床价值,未来有望成为PFCs个体化诊疗的普惠性方案。
