肺炎衣原体肺炎的诊疗新进展：流行病学、耐药机制与治疗策略

摘要

肺炎衣原体（chlamydia pneumoniae，Cpn）是社区获得性肺炎（community acquired pneumonia，CAP）的重要病原体之一，全球感染率达50%~70%。其具双相发育周期，通过直接损伤与免疫病理损伤致病，经呼吸道飞沫传播，临床表现为长期咳嗽、低热等，与哮喘等慢性疾病相关。诊断依赖病原学（培养法、抗原检测）、血清学（MIF、CFT）、分子生物学（PCR、tNGS、mNGS药物显示良好活性，疫苗研发有进展。近年耐药问题凸显，Cpn 可通过 rpoB、gyrA 等基因突变对利福霉素、氟喹诺酮类耐药。未来需深入研究流行病学、致病机制，优化治疗方案，加速疫苗研制。
