腹腔镜胃肠道肿瘤术后行腹腔热灌注化学治疗安全性研究的综述

摘要

腹腔镜胃肠道肿瘤切除术后联合腹腔热灌注化学治疗（hyperthermic intraperitoneal chemotherapy，HIPEC），是近年来在微创肿瘤综合治疗领域中兴起的一种区域性术后干预手段。该技术结合了微创外科的低创伤特性与局部高浓度化疗药物的定向作用，并通过热效应增强药物对残留肿瘤细胞的杀伤力，旨在减少术后种植复发及远期转移。众多文献资料显示，其在术后早期进行可有效阻断肿瘤复发，但同时也存在一定的组织灼伤、感染及腹腔粘连等潜在风险，需在充分评估术后状态的基础上，合理控制灌注温度、时间及药物浓度，以实现疗效与安全性的平衡。本文将围绕其治疗机制、适应风险、主要并发症及安全性文献数据展开系统综述，为规范临床操作路径、优化围术期管理提供理论支撑。
